Our doctor’s are improving their patient care to meet the Federal requirements needed for all insurance carriers. There are many changes being made at our office to ensure your quality of care is maintained and that we meet the all of the new guidelines and requirements.

Appointments: All patients are asked to schedule an appointment to receive medical care. We request that you call in advance to reserve a designated time for you. We realize that your time is valuable, and we will make every effort to honor all time commitments. If you are seriously ill when you come to our clinic, you may ask to be seen immediately. We will do everything possible to accommodate you.
To schedule an appointment, please call 281-580-7004. Please cancel all appointments 4 hours in advance to avoid a potential $25.00 charge. If you are 30 minutes late you may be asked to reschedule. Give us complete, accurate information about your present illness, your medications, medical history and other important facts about your health. Bring in your medications if necessary. If you are a new patient please arrive 30 minutes early to fill out forms.
Walk Ins: due to high volume of referrals, prescription refills and insurance forms required by insurance companies, we have to eliminate walk-in requests. Telephones: We have implemented telephone options to choose from. Please listen carefully and choose the one which services your need. We strive to return voice mail messages as soon as we can, on the same day, if possible.
Charges and Insurance Payments: Houston Northwest Primary Care services traditionally insured patients, private pay, and wide variety of insurance plans including HMO’s PPO’s managed care plans. Patients in an HMO or PPO plan may be required to pay a co-payment or deductible. Patients who are not covered by an insurance pan will be asked to pay for medical services at the time they are rendered. For our insured patients, you are required to bring your insurance card and driver license to each visit, once this is confirmed and verified we will file your insurance claim.
Third Party Insurance: We do not file insurance for auto accident injuries, and we do not file on Workers Comp. injuries.
Pharmacy: Pharmacy: Refills take 24 to 48 business hours.Lab and Radiology Results: Takes 7-10 business days. You will receive a notice in the mail or a call from our office to schedule an appointment to discuss your results with your doctor, if needed. If you have not heard from our office in 10 business days, please call our office to inquire about your test results
Triplicate Prescriptions: There is a $15.00 charge per script.Forms (e.g. FMLA, physicals) Letters and copy of Medical Records: $35.00 Charge. Medical records release must be signed by you or your legally authorized representative.
Hospital Admissions: Should you need to be admitted to Houston Northwest Medical Center, your care will be managed by our hospitalist, Dr. Annamaneni, a critical care specialist.
Referrals: Plan Ahead All patients needing referrals to specialists, need to schedule an office visit to have this done. Contrary to what you may have been told by your insurance company, referrals do take time to process. If you schedule an appointment with a specialist, please do not wait until the day before your appointment. We need 7-10 business days. If you have been to a specialist, please have the specialist mail or fax a completed report on your visit, along with any requests for tests and reason for additional visits. (i.e. diagnosis). We will call you when they are ready.
Physicians Assistant/Nurse Practitioner Credentials: Physician Assistants (PAs) and Nurse Practitioner (NPs) are licensed health professional’s who practice medicine with physicians supervision. Within the physician/PA/NP relationship. PA’s/NP’s deliver a broad range of medical and surgical services. PA’s/NP’s are highly skilled professionals who provide medical services that would otherwise be performed by physicians. PA’s/NP’s take medical histories, perform physical examinations, diagnose and treat illnesses, order and interpret lab tests, perform minor surgery and prescribe medication.
